TRANSITIONAL LIVING CENTRES (TLC) was founded in 1999 to provide community-based residential rehabilitation services for persons with an acquired brain injury. TLCis the leading provider of community reintegration services in Manitoba for persons with acquired brain injury.
Our mission is to provide a safe, supportive environment during the rehabilitation process as residents’ progress in their independent living skills.
Your Opportunity:
We are seeking an experienced case manager to work as a key member of
our team in Winnipeg.
As a Case Manager, you will provide a wide variety of services to our
participants while taking necessary steps to ensure their safety and
well-being. In your role, you will utilize case management processes including critical
thinking, problem solving and decision making, as well as providing instruction,
counsel and advocacy on behalf of our residents. You will play a leadership
role in supporting client care and health promotion. You will assist in
providing safe, quality, client centred care while reflecting the shared vision
and values of TLC.
Required Qualifications:
Applicants must possess the following qualifications to be considered
for this position:

A degree in
Social Work, Occupational Therapy or other directly related human services
field.
·
A minimum of two years related work experience.
·
Possess a valid driver's license and have access to
a vehicle.
·
Have current Criminal Records and Vulnerable
Persons clearance.
·
Possess excellent oral and written communication
skills and a knowledge of issues related to the rehabilitation of persons with
acquired brain injury.
·
Show evidence of the ability to work as part of an
interdisciplinary team and coordinate resources on behalf of our clients.
·
The successful
applicant will be a "hands on" person who may be required to work
some evenings and weekends and provide on call support on a scheduled basis.
